What to Expect

Before Your Visit

When you call to schedule an appointment, our professionally-trained administrative staff will ask you a few questions about who you are, what might bring you to counseling, and basic insurance information. From the information you provide, we will identify a therapist who can best meet your needs and schedule your first session with a counselor.

As a new client, we will request that you complete paperwork which will allow your therapist to provide treatment that meets your individual needs. Each individual is required to complete the paperwork prior to their initial appointment. In the event that you are a child (under 18 years of age), we ask that a parent or guardian sign all forms.

The Psychosocial Assessment form can be completed prior to your first visit. In order to obtain the proper information from the Psychosocial Assessment, there are three different versions available. Please bring the form with you on your first visit.

CHILD ASSESSMENT
(AGES 0-11)

Anyone under 18 years of age must have a parent or guardian sign the form.

ADOLESCENT ASSESSMENT
(AGES 12-17)

Anyone under 18 years of age must have a parent or guardian sign the form.

ADULT ASSESSMENT
(AGES 18 & UP)

Please bring all forms to your first visit.

When You Arrive

Please arrive twenty minutes before the appointment time to complete the remaining paperwork. Our reception staff will be happy to review each of the required forms with you and answer any questions you may have. Our goal is to make the intake process as stress-free as we can.

Also, please have your primary care physician's information. We encourage communication with your primary care physician as it allows our agency to ensure you are receiving proper care; however, it is ultimately your choice to release information to your primary care physician. Please discuss any questions or concerns you may have regarding this process with your therapist. .
