As a new client, we will request that you complete paperwork which will allow your therapist to provide treatment that meets your individual need. Each individual is required to complete this paperwork prior to their initial appointment. In the event that you are a child (under 18 years of age), we ask that a parent or guardian sign all forms.
The Patient Information and Psychosocial Assessment forms can be completed prior to your first visit. In order to obtain the proper information from the Psychosocial Assessment there are three different versions available. Please choose from the following:
If you choose, you may complete the forms and bring them with you to your first appointment. Please do not sign these forms prior to your appointment as a witness signature is needed. Also, on the Assessment form, please do not write anything in the large boxes as these will be used by your clinician. Once you arrive, our reception staff will be happy to review each of the required forms with you and answer any questions you may have.
When arriving at the office, please ensure you have your primary care physician's
information. We encourage communication with your primary care physician as
it allows our agency to ensure you are receiving proper care; however, it
is ultimately your choice to release information to your primary care physician.
Please discuss any questions or concerns you may have regarding this process
with your therapist.
